Health authorities confirmed the case was one of five new regional cases on Sunday.
There was also single cases recorded in Geelong and Mildura, while two new cases have been listed in East Gippsland.
Deputy Chief Health Officer Dan O'Brien said the Macedon Ranges case was linked to the V/Line outbreak.
Four workers at the regional train service have tested positive to coronavirus.
"We would prefer it was not the case but you cannot ignore it got into the (V/Line) system," Premier Daniel Andrews said.
"Many have to work hard to support the people through testing and isolating and we get services back up and running as soon as we can."
It comes after a number of exposure sites were listed in central Victoria on Saturday evening.
The Ravenswood Caltex service station was listed as a tier two site, along with a Woodend liquor store and another service station in Carlsruhe.
Premier Daniel Andrews said 85 per cent of the state's 3112 active cases were people aged under 50.
There were 147 people in hospital, with 34 of those people in intensive care and 28 on ventilators.
Mr Andrews said only one fully vaccinated person was in hospital, with that patient not receiving intensive care.
The premier urged people to register to get vaccinated. He said there were 7000 Pfizer appointments and 11,000 AstraZeneca appointments available at state-run clinics this week.
