Bendigo Highway Patrol's Senior Sergeant Ian Brooks said initial investigations indicated that a sedan had crossed over the centre of the road and collided with the truck late yesterday afternoon.
"We believe that a sedan travelling southbound on Axedale-Goornong Road in Axedale crossed onto the incorrect side of the road," he said.

"The sedan collided with a truck which was travelling in the opposite direction."
Senior Sergeant Brooks said the occupants of the sedan were able to get out of the vehicle before it was engulfed in flames.
He said one of the occupants was taken to Bendigo Hospital by road and the driver was airlifted to Melbourne with serious but non life-threatening injuries.
Senior Sergeant Brooks said the driver of the truck received minor injuries.
7pm:
FIVE people have been hospitalised after a car collided with a truck near Axedale late this afternoon.
The silver Holden Commodore was travelling from Echuca when the crash occurred at roughly 5.15pm.
The car burst into flames after everyone had escaped.
One man aged in his 50s sustained serious but non life-threatening injuries and has been airlifted to the Royal Melbourne Hospital, police and paramedics have confirmed.

Paramedics assessed and treated two other adults and two teenagers at the scene, Ambulance Victoria said.
Both teen boys were taken to Bendigo Hospital in a stable condition. One boy has lower body injuries, the other was transported for observation.
Ambulance Victoria said a man in his 40s was taken to Bendigo Hospital with lower body injuries in a stable condition and another - whose age is yet to be confirmed - was sent to the same place with minor injuries.
Firefighters have declared the incident under control but police expect the road to remained closed in both directions for at least another hour while a clean up takes place.
6pm:
AXEDALE-GOORNONG Road is closed in both directions and emergency service members are turning cars around following a multi-vehicle crash.
Traffic is banking up along the road, including a lot of workers from the Fosterville Gold mine.
An air ambulance has just landed at the scene, north of Axedale.

It is not yet clear how many people have been injured, or how many vehicles were involved.
Firefighters are yet to declare the incident under control.
One car is still burning.
Earlier
EMERGENCY crews have arrived at the scene of a multi-vehicle crash near Axedale.
The Bendigo Advertiser understands no-one is trapped in any vehicles but at least one is alight on Axedale-Goorning Road.
Multiple CFA appliances are en route to join others already at the scene.
Paramedics and police have also been called.
It is unclear whether the road will need to be blocked.
