Goulburn Valley Health chief executive Matt Sharp said thousands of residents were currently isolating.
He said the 21 positive COVID cases were mainly related to three closely associated family groups and their extended networks.
He said it was not yet known how long the people had been infectious.
A Bendigo business has been listed as a tier two exposure site after a COVID-19 positive person visited on Friday August 13.
Mr Sharp said the health department was still investigating where the initial case contracted COVID-19.

He said about 100 Goulburn Valley Health staff were furloughed as they were linked to the schools which were exposure sites.
"We are fairly challenged with staffing numbers at the moment," he said.
Additional resources from Melbourne and Bendigo Health had been called in to support them during the outbreak.
More than 1000 tests had been performed today, with long waiting times at testing clinics.
Bendigo Health testing is available on McLaren Street, both drive-through and walk-in, from 10am to 5.30pm seven days a week.

Tier 1 sites
Tier 1 - Get tested immediately and quarantine for 14 days from exposure
People and household contacts from these classes are considered a Tier 1 exposure and should be tested for COVID-19 and quarantine.
- GSSC McGuire Campus. - Teachers and student in year 7, year 11. Present to ARC GV Health Graham Street for testing.
- St Mel's - Teachers and student in prep, year 3/4. Present to Shepparton Sports Precinct for testing.
- Orrvale Primary School - Teachers and student of year 5/6. Present to Shepparton Greyhound and Trotting Track for testing.
All other students, teachers and household contacts from the schools above should quarantine at home and keep safe until you are notified by the contact tracing team.
