The proposed merger requires a vote by Cobaw members at a Special General Meeting on September 23, in order to proceed.
The joint venture would expand opportunities for health and community services across Sunbury and the Macedon Ranges, according to a joint media release issued by the two community health services.
There is expected to be little change for clients, frontline staff and volunteers, with the two organisations currently undertaking similar roles.

In May, Cobaw Community Health moved to a new facility at Caroline Chisholm Drive in Kyneton after operating at its High Street premises for more than 25 years.
Staff are better accommodated at the site, Cobaw chief executive Margaret McDonald said in May.
"Our staff spaces are now completely separate from client spaces," Ms McDonald said.
