Golden Point, the Ballarat T20 premiers, defeated White Hills by six runs in a thriller.
After being sent into bat, Golden Point was 3-106 with four overs remaining.
Jacobs changed the momentum of the game in the space of four balls.
With the first ball of his final over he had Golden Point star Manjula De Zoysa caught for 51.
Next ball he trapped Daniel White lbw for a golden duck before completing the hat-trick when he clean bowled James Lewis.
For good measure, Jacobs made it a double hat-trick when Andrew Falkner was given out lbw.
Jacobs, who finished with figures of 5-14, also took a hat-trick for White Hills in the Northern Rivers leg of the Regional Big Bash earlier this summer.
From 3-106, Golden Point crashed to 9-107 with 14 balls remaining.
White Hills had an opportunity to dismiss Golden Point for 108, but a catch went begging and the visitors made the Demons pay.
Simon Ogilvie (32) and number 11 Nick Smith added 31 off the final 14 balls to give Golden Point a good total of 138.
Their final over haul of 18 included a dropped catch on the boundary line that went for six.
In reply, White Hills skipper Mitch Winter-Irving and Rhys Irwin gave the home side a bright start.
They added 62 before Winter-Irving fell for 29.
Irwin and Gavin Bowles had the Demons well placed at 1-84 in the 11th over before Bowles fell for 15.
Irwin and Tom Dunham took the score to 2-110 with five overs remaining before the home side fell away.
In the next three overs the Demons lost 2-10 and control of the game.
With two overs left they needed 19 runs to win, but they couldn’t find the boundary against the disciplined Golden Point attack and the vistors won by six runs.
Irwin top scored for White Hills with 50 off 52 balls.
Golden Point went on to defeat Swan Hill in the semi-final stage on Sunday afternoon to advance to the finals in Melbourne next month.
