The man, in his 70s, sustained pelvic and abdominal injuries after the vehicle he was driving collided with another car near Harcourt.
The man made a U-turn onto the freeway from the access ramp at Harcourt and drove the wrong way down the road for a short time before colliding with a south-bound vehicle about 3.15pm.
His passenger was treated on scene for minor injuries, and four other people were transported to Bendigo Health. The Calder Freeway was closed south-bound for about an hour while the scene was cleared.
