Fire crews and the SES attended the rescue operation after the man's vehicle rolled after hitting a power pole about 1am on Saturday.
The man, in his 40s, was flown to The Alfred hospital where he remains in a stable condition.
Police have confirmed an off-duty nurse helped at the scene of the crash.
A Castlemaine police spokesman said the male driver of the car was unconscious but breathing when emergency crews arrived.
"The power pole snapped across the north-bound lane, the Midland Highway," he said.
"The pole was down for three or four hours."
The cause of the crash is unknown.
The crash comes after police conducted a road safety blitz near Tylden last week.
