Emergency crews rushed to the head-on accident on the Maryborough-Dunolly Road just after 2pm.
A female passenger in one car has been airlifted to The Alfred hospital in Melbourne, while the sole occupant of the other car was taken by road to the Maryborough Hospital.
Police are investigating the cause of the accident.
"One was going to Dunolly and one was going to Maryborough and they crashed," Central Goldfields Highway Patrol senior constable Brian Smith said.
The deceased man is believed to be from the area.
The Maryborough-Dunolly Road is closed in both directions between Bromley-Timor Road and Bet Bet-Betley Road.
Motorists are urged to avoid the area.
