Strathfieldsaye resident Timothy Werps appeared in the Bendigo Magistrates' Court on Wednesday.
He formally entered guilty pleas to charges of culpable driving causing death and negligent driving causing serious injury, after he last month indicated his intention to plead.
The offences stemmed from the fatal car crash on McCrae Street in Bendigo on September 28 last year.

Police alleged at a bail hearing last December that Mr Werps was the one driving the Holden Commodore that slammed into a pole and a tree.
Rear passenger Jye Eiffert, who was just weeks shy of his 17th birthday, was thrown from the vehicle and died at the scene, while an 18-year-old front passenger sustained serious injuries.
The court was told Mr Werps drank alcohol and smoked cannabis before getting behind the wheel.
Multiple CCTV cameras allegedly captured the 19-year-old travelling at a fast speed down the Midland Highway, Napier Street, and McCrae Street, before he lost control of the car and crashed it about 3.11am.
Mr Werps will appear in the County Court in September for his plea hearing.
The 19-year-old's bail was extended to his next court date. As part of his bail conditions, Mr Werps was ordered to report to police once a week and follow an 8pm to 7am curfew.
