Bendigo Sergeant Mick McCrann said the two-car crash at Epsom happened when the driver of a black ute crossed onto the opposite side of Howard Street and hit an oncoming white Mitsubishi.
"At this stage, one vehicle is on the incorrect side of the road," he said.
"The evidence is clear and we have a witness attesting to that. So obviously there is some fault on that driver and we will be following that up."
Sergeant McCrann said the male driver of the black ute, 32, suffered leg injuries. An ambulance took him to Bendigo hospital for treatment.
The 55-year-old male and 46-year-old female, both of Huntly, were also injured and treated by paramedics at the scene.
"At one stage we had SES, fire, police and ambulance on scene," Sergeant McCrann said.
"We needed some assistance in getting people out of the vehicles, so they were trapped for a little while."
Police are investigating the incident.
"We would appeal to any witnesses who may have seen the incident occur to contact police or highway patrol at Bendigo," Sergeant McCrann said.
Tow trucks have now removed the cars from the scene and police and the Country Fire Authority is preparing to reopen Howard Street.
