A DEATH at a Bendigo aged care facility has been referred to the coroner, as auditors confirmed a medication error led to a "significant negative outcome" for one resident.

The Aged Care Quality and Safety Commission said that a recent death at Royal Freemasons Flora Hill had been referred to the coroner, in response to questions about the outcome referred to by auditors.
The incident was referenced in the commission's latest audit into the home, which found it non-compliant with quality and safety standards.

Assessors cited issues such as medication errors and staff shortages.
They said there were "deficiencies" in management's response to a resident receiving the wrong medication.
The commission told the Bendigo Advertiser that it became aware that a recent death of a resident had been referred to the coroner for investigation when conducting an audit at Royal Freemasons Flora Hill.
The commission warned it was closely monitoring Royal Freemasons in Flora Hill to make sure the home took all the necessary steps to improve the safety and quality of care.
The commission warned it had a number of further regulatory actions it might consider if a non-compliant service was not meeting its obligations.
These option include applying sanctions, issuing a notice requiring agreement to certain actions, varying accreditation status, or revoking an approved provider status.

The commission said the unannounced site audit at Royal Freemasons Flora Hill took place between March 15 and 17, after the commission received complaints about the facility.
The side audit found the service was non-compliant with three of the 42 requirements of the Aged Care Quality Standards.
These were in the areas of personal and clinical care, human resources and organisational governance.
The commission said it issued a note of non-compliance to the service on June 8.
Royal Freemasons Kangaroo Flat was also found non-compliant with several aspects of aged care quality and safety standards, just months before the report into Flora Hill.
A Royal Freemasons spokesperson said they would like to reassure the community, residents and families, that its Flora Hill home remained fully accredited by the Aged Care Quality Commission.
They said managers and staff continued to focus on the non-compliances identified by the commission, to make sure its residents received the highest level of care.
