The shire's survey will allow residents to share how they have coped with the COVID-19 pandemic, as well as how the National Disability Insurance Scheme has affected their lives.
Survey responses will be confidential or anonymous, and will only be used to develop the Disability Action Plan.

Central Goldfields Shire chief administrator Noel Harvey said the Disability Action Plan would aim to make the community even more inclusive.
"We know that we can always do better and so we are keen to hear from people living with disability about what we can try to improve," he said.
"Whether it's limited community transport options, problems accessing places in mobility scooters or wheelchairs, or difficulties in gaining employment, you can now have your say.
"We are also keen to hear from people with non-physical conditions about what we can do to make life better for them.
"There many different forms of disability and we want to take them all into account to make life as fulfilling as possible for as many people as possible."
Disability services provider Asteria chief executive Annie Constable also encouraged people living with disability and carers to complete the survey.
"Sharing your thoughts and experiences by completing the survey is a valuable way to help Council shape its plan and work towards making our Shire inclusive and accessible for all," she said.
The survey is available at centralgoldfields.vic.gov.au/haveyoursay and closes on September 18.
