The 58-year-old, from Tooborac, was a passenger in one of the vehicles.
He was airlifted to a hospital in Melbourne with life-threatening injuries, but died in hospital on Saturday.
The man was one of five people injured in the crash, which happened on the Northern Highway near Seymour-Tooborac Road shortly before 5.30pm on January 21.
Police were told three cars collided, one of which was towing a horse float.
Four people were taken to hospital with non-life-threatening injuries.
They included a 54-year-old Tooborac woman, a 12-year-old Tooborac girl, a 46-year-old Heathcote man, and a 41-year-old Seymour man.
Major Collision Investigation Unit detectives are investigating the circumstances surrounding the crash.
Thirty-four people have lost their lives on Victorian roads so far this year, one more than this time last year.
