WorkSafe wants Strathfield Village Supermarket wound up, a liquidator appointed and costs to be paid.
A statutory demand for payment of debt, served on the company on June 3 and filed with the Supreme Court, says the company owes WorkSafe $91,510.64.
The debt relates to an unpaid insurance premium of $85,038.82 for the year 2018-2019 and late payment penalties of $6471.82.
The demand stated payment was due within 21 days, but it is claimed Strathfield Village Supermarket failed to comply.
The affidavit of a workers' compensation agency employee states he inspected the business records of WorkSafe in relation to its account with Strathfield Village Supermarket and the debt was due and payable.
He also states in the affidavit that he believes there is no genuine dispute about the existence or amount of the debt.
Another document filed with the Supreme Court gave notice that Energy Australia, another creditor of Strathfield Village Supermarket, was prepared to support the application for the winding-up of the company.
The document says Strathfield Village Supermarket owes Energy Australia $620,719.90.
The matter was brought before the Supreme Court last Wednesday and was adjourned to later this month.
A court order noted the directors of Strathfield Village Supermarket were facing a "significant health crisis" and while the plaintiff, WorkSafe, had sought a six-week adjournment, the supporting creditor was owed a significant debt.
It also noted that affidavit material did not disclose how the company intended to deal with the proceeding.
No representative for Strathfield Village Supermarket appeared at last week's hearing.
