Scores were locked at 0-0 at half-time mark, with the Blazers being unable to capitalise on their opportunities in the first half.
After the break, the Blazers came out firing, maintaining possession and linking up well across the field.
The midfielders created many chances for interceptions and quick turnovers stretched the Knox defence.
Kate Robertson scored a double for Blazers with Josien Lensen and Tracey Johnson each scoring one.
The Blazers will now move into the decider to face powerhouse St Kilda in the Vic League 1 grand final this Sunday 2pm at the State Hockey Centre.
