A young Country Fire Authority volunteer kicked and assaulted by four male members of her brigade was also duct-taped to the front of a fire truck in a separate incident, an official investigation has revealed.

Cultural problems, alcohol abuse and a lack of discipline and respect have been blamed for a series of assaults at the Eaglehawk Fire Brigade in Bendigo in 2017.
An investigation sparked by the assault of the 17-year-old female volunteer in November found "a pattern of poor behaviour" at the station in addition to significant leadership problems.
The report was made public on Wednesday, nearly three months after CCTV footage revealed the girl was kicked, dragged and pulled by the hair by several Eaglehawk CFA members while station captain Hayden Allen failed to intervene.
Mr Allen and four members were suspended in the wake of the assault which was initially portrayed as "play fighting" by those involved.
"It has become apparent that the incident on November 27 was not an isolated event, but was in fact the latest in a series of inappropriate actions directed at the young woman and other young members of the brigade," the report said.
"Evidence has been provided of three other recent incidents of a similar nature, all involving potentially harmful physical contact with young people.
"The captain of the brigade was involved in all four of the incidents."
The report found that there were cultural problems at the brigade, including a lack of discipline and respect; that the consumption of alcohol contributed to at least one of the incidents; and that the brigade had been let down by significant leadership deficiencies by its then-captain, who was involved in a number of incidents.
On one occasion, members were filmed drinking heavily inside the Eaglehawk Station and throwing their colleagues into a water trough. One volunteer was filmed with a bottle of scotch in one hand and a can of beer in the other.
Mr Allen has since resigned from the organisation but would have been the subject of "serious disciplinary action" had he remained a CFA member.
"It is vitally important that cultural problems inside the organisation are addressed and young CFA volunteers are supported and encouraged to continue the good work of CFA," a statement from the authority said.
"CFA must collectively take a stand and work together to ensure that we grow into a modern and inclusive organisation for the future."
A series of recommendations include the appointment of a CFA staff officer to review leadership positions within the brigade and urgent retraining on CFA values.
An overhaul of the CFA volunteer member selection process was also recommended in addition to the prohibition of alcohol consumption at the Eaglehawk Fire Station.
In October, The Age revealed an internal report that exposed bullying and sexual assault in the ranks of the CFA statewide.
Women reported that they had been sexually harassed and bullied and ignored if they complained about their treatment.
