Paramedics could soon have access to a smartphone app that will track the location of patients from the scene to hospital after difficulties emerged in dealing with the Bourke Street aftermath.

Emergency services were confronted with an unprecedented amount of victims spread over a large area when Dimitrious Gargasoulas allegedly drove a car into pedestrians on Bourke Street in January last year, killing six people and injuring dozens more.

After the incident, Ambulance Victoria produced a report with 33 different recommendations on how to deal with a similar incident in the future, including a new app called "Track Me" to help paramedics respond to patients by their location, as well as their condition.

The system is being trialed in the Barwon region and could be extended statewide later this year.

Ambulance Victoria state health commander Paul Holman, who was at the Bourke Street scene managing paramedics, said that one of the difficulties had been not knowing exactly where each patient was at all times.

During Bourke Street, he said all the dispatches that came through were consolidated into one job, which made it hard to track in real time.

"Because I couldn't see the patients we had no idea what we were dealing with," he said.

The new system will mean that when paramedics initially triage a patient, a barcode placed on their wrist can also be scanned to record their location, identity and condition to a central emergency system.

Each time a patient is moved or their condition changes, the barcode can be rescanned to update those who are trying to manage the overall response.

Mr Holman said he had not come across a scene like Bourke Street in his 42 years working for Ambulance Victoria, where all the victims were so spread out over a scene more than a kilometre long.

"They were in laneways, they were in shopfronts, they were over the hill," he said.

"The challenge was just daunting, it was a matter of trying to get some sort of picture of what happened and where everyone was."

Another initiative, which was implemented at the recent Flinders Street car attack, is to allocate a manager to provide counselling support for paramedics at the scene of major trauma cases.

Mr Holman said that Ambulance Victoria employed a large workforce of paramedics, many of whom were young and needed regular support.

"We're not superhuman, we're not heroes. We're just doing the job and we're doing it in unusual and difficult circumstances," he said.

"I've never met a paramedic who says that you get used to what we do."
