The collision occurred at the intersection of Epsom-Barnadown Road and Axedale-Goornong Road about 12.20pm on December 21.
The 71-year-old driver from Boondall, Queensland, was the driver of a silver Ford sedan when it crashed into a black Ford four-wheel drive.
He was airlifted to hospital with life-threatening injuries and has since died in hospital.
His female passenger sustained serious injuries and was also taken to hospital for treatment.
The two occupants of four-wheel drive sustained minor injuries and were treated at the scene.
At the time, Bendigo Sergeant Mick McCrann said a silver Ford carrying a Queensland couple was travelling on Epsom-Barnadown Road when it crossed the Axedale-Goornong Road intersection and struck a vehicle heading north. He said it appeared the sedan driver was unfamiliar with the road.
Bendigo Highway Patrol members are investigating.
